Payment History Report

We have patients requesting payment history for the past year for tax purposes and we're finding there is not a nice looking report to give to patients at all. We've tried different options within the statements, but that always includes full account activity and not just a payment history. We've also pulled the Activity Report set for 2025, with only Other Payments selected and just for that patient. That report give us exactly what we need when viewed under the Detailed tab, except when you export the actual PDF report it looks terrible to give to a patient. It doesn't have the practice name or even use decimals or $ for the payment amounts that do not have cents. We really need a clean, easy to pull option that looks professional to give to patients.
