appointment category button to be more obvious/for online schedule to first prompt patient to choose a category

Almost every new patient that schedules online, schedules as a recurring patient/adjustment. It's because the select category isn't obvious. I think this need to be more "in your face" or a button to choose before the calendar appears. Right now the categories tab is very easy to miss.
