Improve Template Visuals

1.) Text boxes within a note aren't obvious to see if there isn't data in the field (see SS 1).

2.) Also, the subluxation levels aren't easy to see if picked. See SS 2. An idea would be to make them grey so they someone who might have vision issues can clearly make out what is chosen.

3.) Also, another feature would be a "clear" button so they reset instead of clicking each one off and clicking new ones on.
