Update Appointment Request Page

Allow offices to customize the appt req by turning off the desired appt day/time to make it a pure contact form. Allow them to add fields for patient's to complete. Put these in their own request bucket (contact form completions). Would be great to update the desired day/time if they choose to keep to more open ended options.

Choose Day(s) of Week Prefer: Then the patient can select box(es), Days of week Checkboxes Monday, Tuesday... And then time of Day Preferred: The patient can select box(es) for time of day preferences, Morning, Lunch, Afternoon, Evenings
