Easily Insert + Remove Macros by clicking to save time

Inserting macros into notes - could save a lot of time if dr. is able to simply click the macro to insert and then click it to remove versus having to go back into the text and highlight/erase or clear all to remove.

Improve Template Visuals

1.) Text boxes within a note aren't obvious to see if there isn't data in the field (see SS 1). 2.) Also, the subluxation levels aren't easy to see if picked. See SS 2. An idea would be to make them grey so they someone who might have vision issue...
